Prevalent Student Medical Conditions (click for hdsb.ca forms)

The Halton District School Board has updated several Administrative Procedures to better support students who have Anaphylaxis, Asthma, Diabetes, Epilepsy and Seizure Disorders. With these revised procedures students who have any of the listed prevalent medical conditions must have a Plan of Care in place and Request to Administer Medications forms signed by a physician. Please contact the office at 905-827-2741 if your child has a prevalent medical condition.

2020-21 Fundraising Summary - Due to the unusual nature of last year, we have no fundraising to report for this past school year.

RETURN TO SCHOOL AFTER THE HOLIDAYS

Happy New Year, Brookdale families. We are awaiting further instructions from the Ministry and the Halton District School Board on moving forward after the Winter Break. Please monitor your emails for upcoming messages regarding the return to school. If you require the Student Daily Screening as it was misplaced over the holidays the school will have extras the first day back.
